When you book an appointment you will receive a confirmation e-mail with all the information you should need, along with a questionnaire covering your general health and medical history, please could you complete and submit this before your appointment. 

Should you need to cancel or change your appointment there will be a cancellation link on your confirmation and reminder emails, or get in touch.  Please try to give us at least 24 hours notice. For appointments cancelled with less than 24 hours notice we reserve the right to charge the full cost of the appointment and/or refuse future bookings.
